    3. Prescription grade oral medication administered in the clinic. (e.g. Clonidine, Prednisolone, Zofran, etc).

    4. Intermediate level wound evaluation and suture or staple removal that was initially performed at an outside facility.

    2. OTC equivalent oral medication administered in the clinic.

    3. Prescription order, if indicated.

    4. Physician referral, if necessary.

    5. Simple level suture or staple removal and wound evaluation that was initially performed at an outside facility.

Intermediate

No co-payment. No deductible. No coinsurance.

Includes all listed on the Simple2 Service.

        Plus:

    1. Lab specimen collection and handling. (e.g. PCR, Blood, Culture, etc.) Note: Lab specimens are sent to our affiliated external lab listed below. Charges per lab test/order will apply at a low cost.

      1. Quest Diagnostics.

      2. LabCorp.

      3. Clinical Reference Lab (CRL).

    2. Radiologic Imaging. *No CT, US, MRI, PET Scans, or other advanced imaging in-house.

      1. X-Ray ($25 fee for each additional body imaging)

      2. ECG/EKG

    3. Earwax Removal via Irrigation and/or Manual.
    4. **Motor Vehicle Accident Evaluation.

    5. Subcutaneous and Intramuscular Injection Medications.

    6. Nebulizer Treatments.

    7. Hook and Loop Elastic Bandage Wrapping. (Crutches, slings, walking boots, post-op shoes, and other DMEs are sold separately).

    8. Complex level wound evaluation and suture or staple removal that was initially performed at an outside facility.

**We DO NOT ACCEPT automobile insurances at this time. For reimbursement: Most times you will be able to submit the receipt of payment to the insurance company or third party agency or they can request a medical record release with billing by sending the request form via fax with the patient’s authorized signature.

Includes all listed on the Intermediate service.

       Plus:

    1. Procedural Services.

      1. Laceration repair (Sutures, Staples, Dermabond)

      2. Incision and Drainage of Abscess

      3. Foreign Body Removal (SubQ, Ears, Nose, Eyes, etc.)

      4. Wound Care

      5. Anaphylaxis Treatment.

      6. Splints (Crutches, slings, walking boots, post-op shoes, and other DMEs are sold separately).

    2. Up to 1 Liter IV Hydration. (Nutrient IV Therapy not included)

      1. Normal Saline or Lactated Ringers Only.

      2. Additional Bag Cost: 500 mL = $25 and 1L = $50.

    3. Follow up visits within 7 days of initial visit for clearance to return-to-work, school, or activity (requires evaluation in person or via telehealth) and wound evaluation and/or suture removal initially performed by APT URGENT CARE are $60 per visit excluding diagnostic testing or medication administration.

THAT ALL ARE WITH NO SURPRISE BILLS!

    1. That you would never pay more than $300 for your visit per person under standard (usual) circumstances.

      Meaning:

      1. No bills from us sent to you after your visit is completed.

      2. There will only be additional charges where applicable during your visit.

    2. That you will be afforded the same optimal urgent healthcare visit given to insurance policy holders.

    3. That Youth sports physicals are $40.

    1. That All adult physicals (DOT, Pre-Employment, Pre-surgical, Academic, Etc.) are $110. *If blood/specimen lab collection is needed, the visit will require an additional $50 plus the cost of each lab test.

    2. ECG, blood specimen lab draw, and X-ray with outside facility orders starts at $50; $25 for additional images.

Urgent Care Easy Pass

Pay-As-You-Go Membership

Designed for patients who prefer flexibility and only need occasional healthcare services. Members pay a nominal monthly membership fee to access apt+ services at discounted rates, paying per visit or procedure as needed. This option suits individuals with infrequent healthcare needs, those on a tight budget, or those who prefer paying out-of-pocket for specific services without committing to a larger monthly plan and the self-pay (Out-of-Pocket) non-membership option.
