APT’s Affordable Four - $Simple1, $Simple2, $Intermediate, and $Complex

Also known as the S2-I-C-K Services – It’s a “sick” service for the “sick and injured person!”

No Insurance? No problem! Not required.

Simple2

    1. No co-payment. No deductible. No coinsurance.

    2. Includes all listed on the Simple1 Tier.

    3. Includes Provider Consult for sick and injury visits.

    4. Includes 2 of any 9 Point-of-Care Lab Testing. ($20 each for additional tests.)

      1. Rapid Flu

      2. Rapid Strep

      3. Rapid COVID-19

      4. Rapid RSV

      5. Rapid Mono

      6. Glucostick

      7. Urinalysis

      8. Urine Pregnancy

      9. BV/Trichomonas

    5. Includes prescription grade oral medication administered in the clinic. (e.g. Clonidine, Prednisolone, Zofran, etc).

    6. Includes intermediate level wound evaluation and suture or staple removal that was initially performed at an outside facility.

    7. Flat rate of $160. ($20 each for additional POC lab tests.)

Simple1

    1. No co-payment. No deductible. No coinsurance.

    2. Includes Provider Consult for minor problems.

    3. All adult physicals (DOT, Pre-Employment, Pre-surgical, Academic, etc.)

    4. Includes OTC equivalent oral medication administered in the clinic.

    5. Includes prescription order, if indicated.

    6. Includes physician referral, if necessary.

    7. Includes simple level suture or staple removal and wound evaluation that was initially performed at an outside facility.

    8. Flat rate of $120.

Intermediate

    1. No co-payment. No deductible. No coinsurance.

    2. Includes all listed on the Simple2 Tier.

    3. Includes lab specimen collection and handling. (e.g. PCR, Blood, Culture, etc.)

      1. Note: Lab specimens are sent to our affiliated external lab at:

        1. Quest Diagnostics.

        2. LabCorp.

        3. Clinical Reference Lab (CRL).

*Charges per lab test will apply at a low cost.

    1. Includes Radiologic Imaging. *No CT, MRI, PET Scans, or other advanced imaging in-house.

      1. X-Ray (1-3 views)

      2. ECG/EKG

    2. Includes **Motor Vehicle Accident Evaluation.

    3. Includes Injectable Subcutaneous and Intramuscular Medications.

    4. Includes Nebulizer Treatments.

    5. Includes Hook and Loop Elastic Bandage Wrapping. (Crutches, slings, walking boots, post-op shoes, and other DMEs are sold separately).

    6. Includes Complex level wound evaluation and suture or staple removal that was initially performed at an outside facility.

    7. Flat Rate of $200. ($25 fee for each additional body part imaging).

**We DO NOT ACCEPT automobile insurances at this time. For reimbursement: Most times you will be able to submit the receipt of payment to the insurance company or third party agency or they can request a medical record release with billing by sending the request form via fax with the patient’s authorized signature.

Complex

    1. No co-payment. No deductible. No coinsurance.

    2. Includes all listed on the Intermediate Tier.

    3. Includes Procedural Services.

      1. Laceration repair (Sutures, Staples, Dermabond)

      2. Incision and Drainage of Abscess

      3. Foreign Body Removal (SubQ, Ears, Nose, Eyes, etc.)

      4. Wound Care

      5. Earwax Removal via Irrigation and/or Manual.

      6. Anaphylaxis Treatment.

      7. Splints (Crutches, slings, walking boots, post-op shoes, and other DMEs are sold separately).

    4. Includes up to 1 Liter IV Hydration. (Nutrient IV Therapy not included)

      1. Normal Saline or Lactated Ringers Only.

      2. Additional Bag Cost: 500 mL = $25 and 1L = $50.

    5. Follow up visits for wound evaluation and/or suture removal initially performed by APT URGENT CARE are $50.

    6. Flat Rate of $250.($25 fee for each additional body imaging).

Know - be in the know.

    1. That you would never pay more than $250 for your visit per person under standard (usual) circumstances.

    2. That you will be afforded the same optimal urgent healthcare visit given to insurance policy holders.

    3. That you may request not to see a provider for COVID-19 visits for reasons related to:

      1. Traveling

      2. Return to work or school or sports

      3. Entertainment

      4. Pre-Surgical

*Pricing is $35 for rapid COVID-19 and $120 for PCR COVID-19 testing.

    1. Youth sports physicals are $40.

    2. All adult physicals (DOT, Pre-Employment, Pre-surgical, Academic, Etc.) are $120. *If blood/specimen lab collection is needed, the visit will require an additional $40 plus the cost of each lab test.

    3. ECG, blood specimen lab draw, and X-ray with outside facility orders starts at $50; $25 for additional image.

    4. THAT ALL ARE WITH NO SURPRISE BILLS!
