3. Payment & Cancellation Policies

  • Accepted Payment Methods:
    • Credit/debit cards, ACH transfers, HSA/FSA cards, CareCredit.
  • Billing Cycle:
    • Monthly for EliteCare; Monthly or Annually for PAYG Hybrid Membership.
  • Late Payment Policy:
    • Membership deactivation after 30 days of non-payment.
  • Cancellation Process:
    • Members can cancel at any time via written request.
    • No refunds for registration, annual fees, or paid services.

4. Promotions & Discounts

  • Current Promotions:
    • Initial enrollment special: 50% off registration fee for early sign-ups. Offer valid from January 13th to January 27th (tentative, subject to change based on official launch date).
    • Multi-Member Discount: 10% off applies to the first month of billing per account upon initial enrollment/registration.
  • Referral Rewards:
    • Receive a $50 credit for each successful referral. Credits may only be used towards visits, labs, procedures, and x-rays. Excludes discounted services and equipment such as DME, nutrient infusion therapy, and weight management. Credits do not rollover annually.

Can I change membership plans later?

Yes, upgrades and downgrades are allowed with 30-day notice.

What happens if I need care outside your clinic hours?

Telehealth and walk-in urgent care visits are available after hours, on weekends, and holidays for a nominal fee. If it’s an emergency, dial 911 or visit the ER immediately.

Are vaccinations included?

Common vaccines are included for EliteCare members; PAYG members receive discounted rates.

How are lab results shared?

Results are securely posted to the patient portal with follow-up consultations as needed.

How are referrals handled?

Referrals to specialists are coordinated by our care team and include guidance on insurance or self-pay options.

What if I need an advanced imaging service?

Advanced imaging (e.g., CT, MRI, US) is not included but can be arranged at partner facilities with discounted rates where available.

Can I use CareCredit for all services?

Yes, CareCredit is accepted for most services, including memberships and additional procedures.

Are DME items covered by my membership?

DME is not included but is offered at discounted rates for members.

What happens if I miss a payment?

Membership will be deactivated after 30 days of non-payment. Reactivation requires payment of overdue fees and may include additional administrative costs.

Are there age requirements for membership?

Yes, EliteCare membership covers individuals aged 12 years and older. Pediatric patients (under 12) are seen through urgent care services at a discounted rate.

Can pediatric patients access urgent care services?

Yes, pediatric patients can access urgent care services at a discounted rate. This includes walk-ins, after hours, weekends, and holidays.

What are your clinic hours and when can I schedule an office visit?

Clinic hours are 8am to 8pm Monday – Friday and 9am to 5pm on weekends and some holidays. Office and telehealth visits are from 8:30am to 11:30am, stops during lunch period, resumes from 1pm to 4:30pm Monday – Friday. After-Hour visits are 5pm to 7:45pm Monday – Friday and all day on the weekends and most holidays.

Are point-of-care testing (i.e., in-house rapid lab tests) included in the membership services?

Point-of-Care Testing or POCT such as Flu A&B, Strep A, and SARS CoV2 are not included in the membership services. However, they are offered and performed at a favorable discounted rate per test. See lab pricing sheet.

Are medications included in the membership services?

Most oral medications administered in-house are included in the memberships. Intramuscular (IM) and Intravenous (IV) medications administered in-house are offered and performed at a favorable discounted rate. Prescription medications are written electronically at no additional cost to you (purchases are made at the local pharmacy at a discounted rate).
